Medical Forms Needed

Subject: New Medical Forms needed

Hello all-

It’s recommended that the Medical Forms be updated every year.  I was looking through our medical binder and noticed that the forms are expiring this year.

Please fill it out completely, print it out, sign it, and bring to our next meeting, this Tuesday.  I’ll take the forms and update the binder.
